Choice of Beneficiary

The benefits payable at your death will be paid according to the beneficiary you designate on your Enrollment Form unless you change your beneficiary in writing at a later date. If no beneficiary designation has been made, the benefits payable at your death will be paid to the survivors, in equal shares, in the first of the following classes to have a survivor at your death: your spouse, your children, your parents and siblings. If there is no survivor in these classes, payment will be made to your estate. The member is automatically the beneficiary for spouse coverage.

Accidental Death Exclusions

The MooseVIP Senior LifePLUS Plan cannot pay a benefit for a loss which is caused or contributed to by: suicide, attempted suicide or intentionally self-inflicted injury while sane or insane (in Missouri and Colorado while sane); any war or act of war (declared or undeclared); full-time military service; sickness or its medical or surgical treatment, including diagnosis; bacterial infection except through a wound (in PA: injury) accidentally sustained; participating in a riot, committing an assault or felony; alcohol intoxication, as defined in the state where the accident occurred; taking of any drug, medication, narcotic or hallucinogen, except as prescribed by a physician; (In PA - voluntary taking of any drug, medication, narcotic or hallucinogen, except those prescribed by a physician as medication) operating or riding in any kind of aircraft, except as a fare-paying passenger on a regularly scheduled commercial flight or as a passenger in a transport plane operated by the Air Mobility Command (AMC) of the United States (In PA, travel or flight of an aircraft or space craft or descent from such a craft while in flight or subsequent drowning, if the insured is; a pilot, officer or crew member of the craft; if giving or receiving aviation training or instruction; has duties on or relating to the craft; or is being flown for the purpose of descent from the craft while in flight.) taking of alcohol in combination with any drug or medication or sedative; voluntary gas inhalation or poison voluntarily taken, administered or inhaled (unless prescribed by a physician - PA only); chronic alcoholism (in PA); voluntary inhaling of any kind of gas; except during the course of employment (in PA); riding or driving as a professional in any kind of race for prize money or profit. Exclusions may differ in your state. See your Certificate of Insurance for details.

Life Exclusions

There are no life exclusions except for suicide within the first two years after the effective date of coverage (one year in North Dakota and Colorado). In such a situation, a refund of all premiums will be paid. NOTE: This information explains the general purpose of the insurance described, but in no way changes or affects the policy as actually issued. In the event of a discrepancy between these materials and the policy, the terms of the policy apply. All benefits are subject to the terms and conditions of the policy. Complete details are in the Certificate of Insurance issued to each insured individual and the Master Policy issued to the policyholder. This program may vary and may not be available to residents of all states.

Your Senior LifePLUS benefits will continue as long as premiums are paid when due, you remain a Moose member and the group policy remains in force.
